Do I need all eight?

Not at all. You can start with the Daily Care Log. Add the other care logs on days when there is more to track, such as extra doses or several blood pressure readings. The two doctor's visit pages can be printed when you are preparing for an upcoming appointment.

It is always good to check with the office before you go, since every practice is a little different. It often helps to have a current medication list, an insurance card, a photo ID, and your questions written down. The Doctor Appointment Prep Sheet and Doctor Visit Notes give you a place for the list and the questions, and a week or two of recent care logs can be useful to have on hand.

The medication list is on the Doctor Appointment Prep Sheet. It shows everything the person takes, and you update it only when something changes. The Medication Log is a daily record of each dose as it is given.
