A written record is easier to trust than memory, especially on busy days. It shows what was given and when, and over time it makes it easier to see what was skipped, what was late, and how the person responded.

Every care situation is a little different, but most logs cover the time, the name of the medication, the dose, and who gave it. A notes column helps for anything out of the ordinary.
